continuity of care document (healthness) (CCD)

The Continuity of Care Document (CCD) is a standard for document exchange of patient information. The summary contains most commonly needed relevant patient data about current and past health status.

CCD records are structured to be used by web browsers and patient application programs such as Electronic Medical Record( EMR) and Electronic Health Record( EHR).

The CCD specifications were developed by ASTM and Health Level Seven( HL7). The content and focus of CCD documents were determined by Continuity of Care Record( CCR).
